The fields follow two reference documents. NABH’s entry-level standard (2nd Edition, AAC.7) lists the contents of a discharge summary, asks for advice in language the patient understands without abbreviations like BD or TDS, and asks that it say when and how to get urgent care. The “National Standard for Patient Discharge Summary” published by the TPA Medi Assist adds admission and discharge times, the method of admission, the signing doctor’s registration number and, for maternity, the obstetric score (GPLA).
Sections you leave empty don’t print. The generator does not write clinical advice for you: the warning signs, diet and follow-up are yours to write for the patient in front of you. It does not claim to make a summary “NABH-compliant”, compliance is about your process, not a form.
