Fertilisation & culture
Culture media go in with name, batch and expiry — traceable if anything is ever questioned. Fertilisation is counted, and every embryo is graded on Day 3, 5 and 6 with photos on the record, so the Day-5 decision is made on evidence, not memory.

Vitrification & the tank
Freezing is never a solo act: every straw records who did it and who witnessed it. Storage is mapped exactly as your tank is built — cryocan to canister to goblet to the colour of the cryolock — for embryos and semen alike. Type a name; find the straw.

Andrology
Is andrology taken just as seriously?
Half of every cycle starts at the andrology bench. MedStream treats it with the same structure as the embryology lab — not a free-text notes box.
Semen analysis, the WHO way
Collection method, place and time, abstinence, liquefaction, volume, colour, pH, viscosity, pus cells, fructose, total count, a four-way motility breakdown and morphology — captured as structured fields, printed as a clean PDF.
IUI, husband or donor
Both records carry the washing method, sperm-separation device, ovulation-induction protocol, trigger, indication — and the ART bank, when a donor is involved. Printable, like everything else.
Straight into the freezer

Form F is part of the record, not a Saturday chore — alongside witnessed consents in English and Gujarati. The system is ABHA-ID ready, so national health IDs sit on the same patient record.

The donor register holds the ART bank, donor ID, physical characteristics, vials received and any adverse events — with donor OPU records and the donor's own discharge summary alongside.
