How these are made
Why are they free, and what can a template not do?
Every document here is one that MedStream generates from the patient record, the discharge pack from the admission, the ANC card from the enrolment, consents as versioned bilingual documents, registers as by-products of the cases that exist. Handing you the template is the honest way to show what that means: fill one by hand on a busy morning, then imagine it assembled from fields that were captured during the stay, with nothing re-typed and nothing that can disagree with the register.
A template fixes consistency. It cannot remind the woman who missed her 28-week visit, cannot stop the discharge time on the summary disagreeing with the IPD register, and cannot compute the monthly report from the register. Those are the parts software does, and each guide says so plainly at the end, after the template, not before it.

Are these formats free to use in my hospital?
- Yes. Every document here is free to download, use and adapt for your own hospital or clinic, add your letterhead and registration number, remove what you do not need, and save it as your standard. There is no sign-up, no email gate and no attribution requirement.
Are these the official statutory forms?
- No, and each page says so where it matters. Where a form's wording is prescribed by law, the MTP forms, ICMR Form F, the ART Rules consent forms, we explain the form and link to the notified text rather than retyping it. The downloads here are working formats: discharge summaries, an ANC card, a register layout, a monthly-report worksheet, informed-consent forms.
